How to File a Grievance

• In person, by mail, phone, or email to our Section 1557 Coordinator (contact info above)
• In writing, including a description of the problem and requested resolution
• We will acknowledge your grievance within 5 business days

Resolution Process

• We will investigate all grievances promptly and equitably
• You will receive a written response within 30 days of receipt
• If additional time is needed, you will be notified of the delay and the reason

Record Retention

• All grievance records will be maintained for a minimum of 3 years
