About Intensive Case Management
Intensive Case Management (ICM) at Trust SoCal provides comprehensive, wraparound support that coordinates every dimension of a client's recovery journey, from housing and employment to healthcare, legal services, and community resource navigation. Our licensed case managers work closely with clients to remove barriers to sustained recovery and build the stable life circumstances that support long-term sobriety. ICM is particularly valuable for individuals who have experienced homelessness, involvement in the criminal justice system, severe co-occurring mental health conditions, or significant psychosocial instability that has historically undermined their treatment engagement and recovery sustainability.
Case management at Trust SoCal goes far beyond simply referring clients to external services. Our case managers maintain ongoing contact with clients, coordinate between multiple service providers, advocate for client needs within systems, monitor progress toward personal goals, and adjust the care plan in real time based on emerging challenges and opportunities. We maintain lower caseloads than standard case management, allowing for intensive, individualized attention to each client's unique circumstances.
Our case managers assist with obtaining stable housing, navigating transitional or permanent supportive housing programs, seeking employment or vocational training, accessing healthcare (medical, dental, psychiatric), managing legal issues and criminal justice involvement, addressing financial challenges, obtaining government benefits (SSI, SSDI, food assistance), and connecting with community support groups and recovery resources. Many of our case managers have lived experience with addiction, bringing authentic understanding and credibility to their work with clients.
Intensive case management is integrated with all other levels of care at Trust SoCal. Clients receiving residential treatment, PHP, IOP, or outpatient services can also access ICM support, creating a comprehensive, coordinated system of care. The intensity and frequency of case management contact are tailored to each client's needs, ranging from weekly to multiple contacts per week.
