Understanding Traumatic Brain Injury & Addiction
Traumatic brain injury (TBI) is an alteration in brain function caused by a force to the head that is associated with physical, cognitive, emotional, and behavioral changes. TBI ranges in severity from mild (commonly called concussion) to severe, depending on the force of impact, location of injury, and individual vulnerability factors. Mild TBI may produce temporary confusion and brief loss of consciousness, while severe TBI can result in prolonged coma, permanent cognitive deficits, and profound personality changes. The growing recognition of TBI in sports, military service, domestic violence, and motor vehicle accidents has revealed that TBI is far more common than historically appreciated, with approximately 5.3 million Americans living with permanent disabilities resulting from TBI.
The relationship between TBI and substance use disorders is bidirectional and complex. Individuals with TBI frequently develop substance use disorders as a result of cognitive changes (impaired judgment, reduced impulse control), emotional dysregulation (increased irritability, depression, anxiety), and chronic pain (often managed with opioids). The cognitive deficits associated with TBI -- including executive function impairment, memory problems, and reduced behavioral inhibition -- directly increase vulnerability to addiction. Additionally, individuals with pre-injury histories of substance use, depression, or anxiety have worse TBI outcomes and higher risk for post-TBI addiction. Conversely, individuals with substance use disorders are at higher risk for traumatic head injuries due to accident-prone behavior, risky decision-making, and violence-related injury.
