Coordination of Care

TRUST SoCal believes in treating the whole person. While you are in treatment you will be assigned a Case Manager. This provides complete coordination of care and will allow us to match the appropriate services to your needs as necessary. Coordination of care can address potential gaps in meeting clients interrelated medical, social, developmental, behavioral, educational, informal support system, in order to achieve optimal health, wellness and quality of life.

Complete Wellness

TRUST SoCal values complete wellness. Problems in one area of your life can impact other areas of your life. Improving your overall wellness can benefit your mental health. TRUST SoCal implements SAMSHA Eight Dimensions of Wellness to improve mental and physical health.

Partial Hospitalization Program is the highest level of daytime outpatient care available at TRUST SoCal and provides at least six hours per day of group or individual treatment for primary mental health.
